Preparing for an appointment? See below for a list of items to bring.

If you are a new patient with us please bring in the following:

Current glasses or last glasses prescription
Current Contact lens boxes or Contact lens prescription
Current Insurance cards
Medication list
Identification Card
Patient forms can be completed online or in office

We accept most major insurance plans. Please give us a call or inquire in office.
